Root Cause Analysis (RCA) is the disciplined investigation that finds the true, underlying cause of a problem, not just the symptom, and not just the immediate cause, but the reason the immediate cause was possible in the first place. Without RCA, the same problems recur endlessly.
A machine stops (symptom). The bearing seized (immediate cause). The lubrication schedule was not being followed (deeper cause). There was no standard, and no leader routine to check compliance (root cause). Fixing the bearing solves this instance. Installing the standard and the check prevents every future recurrence.
5 Whys: ask "why" repeatedly until the answer becomes a systemic condition, not another symptom. Fishbone (Ishikawa) diagram: brainstorm possible causes across categories, people, methods, materials, machines, measurement, environment. Fault tree analysis for complex failures. Pareto to focus on the vital few problems worth analyzing in the first place.
RCA fails when the team stops at "operator error" or "supplier issue." Those are almost never root causes, they are the visible layer above a systemic condition that allowed the error to happen. Good RCA keeps asking why until the answer is something the organization can change permanently. And good RCA closes the loop: the countermeasure is implemented, verified and standardized.
