Corrective Action is the permanent countermeasure installed after a defect has been discovered, the change to the process, standard or design that prevents the same problem from recurring. It is the "C" in the 3 Cs framework (Contain, Correct, Countermeasure) and the point at which incidents become learning.
A defect appears. Containment protects downstream. Root cause analysis identifies the underlying condition that allowed the defect. Corrective action changes that condition permanently: a Poka-Yoke installed, a standard updated, a supplier requirement tightened, an operator retrained with an updated method. The action is specific, verifiable and durable.
It addresses the root cause, not the symptom. It is verified, the same conditions no longer produce the defect. It is standardized, the change is documented and taught. It is systemic where appropriate, if the same root cause exists elsewhere, the corrective action applies there too. Anything short of these tests is a temporary fix, not a corrective action.
Formalized in an 8D or A3 format, the corrective action report captures the problem, the containment, the root cause analysis, the corrective action taken, verification of effectiveness, and any preventive action needed to stop similar defects elsewhere. It is the artifact that turns individual incidents into organizational memory.
Corrective action is what separates factories that improve from factories that recycle the same problems. Every defect is an opportunity to make the process a little more robust, but only if the corrective action is real, verified and standardized.
